How to Use It

Fill in the driver and vehicle at the top.
Log each trip as you take it, including the purpose - that column is usually the one that matters for a claim.
Total the miles column at the end of the period.

Does it calculate a reimbursement amount?

No - it records miles only. Rates change by year and by country, so multiplying the total is left to you.

Is this acceptable for tax purposes?

That depends entirely on your own tax authority's rules, which we can't advise on. It captures the fields a mileage record commonly needs; check your local requirements before relying on it.
